Non-Invasive Central Venous Pressure Measurement by Compression Ultrasound—A Step Into Real Life: Thalhammer C, Siegemund M, Aschwanden M, et al. Resuscitation 2009;80:1130–6

In a Swiss intensive care unit, 50 patients’ central venous pressures (CVP) were compared using non-invasive and invasive techniques. The non-invasive technique consisted of compression ultrasound (CU) of a patient’s forearm vein. The forearm was chosen for the non-invasive measurement as studies have found that when a patient is in a supine position, pressures are nearly the same in the cephalic, basilic, and brachial veins compared to the superior vena cava. The researchers investigated: 1) how feasible and accurate CVP measurements by vascular ultrasound were done by physicians with only a short training course; 2) simple ultrasound vs. a high-end duplex ultrasound system for measuring CVP; and 3) CU’s ability to detect respiratory changes in CVP. Abstract: Objectives: The mid-systolic deceleration (notch) in the proximal descending aortic Doppler waveform was reported to be common in patients with aorto-iliac disease. However, evaluation of the descending aorta is limited to echocardiography and may be technically difficult. Therefore, we decided to check whether similar Doppler flow disturbance can be found in abdominal aorta, which is easily evaluated in wider range of patients undergoing general abdominal and vascular ultrasound, as well as echocardiography.Methods: We evaluated 115 consecutive symptomatic patients with severe peripheral artery disease admitted for vascular surgery, and 60 controls. As a medical community, we continue to “tighten our belts” as a result of the declining reimbursement from the Center for Medicare and Medicaid Services (CMS) for medical imaging— a consequence of the Balanced Budget Act of 1997.
